NOTE:
To submit a new appointment please fill out the complete form to the right. Once you have sent the appointment to us, you should receive an email within 24-48 hours of submitting your appointment. This will let you know if you have a secure date or if your appointment is on Standby for the day! You must fill out a form on each appointment you submit!
You will also receive a confirmation text the night before your appointment from our office to make sure you have checked in with your referrals and confirmed that the appointment was still good and that both spouses could attend the entire presentation. Please make sure and respond to that confirmation text asap or let us know at any time if your appointment has to reschedule.
